1. Complete sections A and B of this nomination form.
2. In SECTION B - NOMINATION CRITERIA, you need to provide examples.
3. Eligible employers/nominees must be a business in Surrey, Delta, or White Rock.

If my employer (or nominee) is selected, what happens next?
You will be contacted to ask for a copy of their business logo or photo of the employer and the employee together in the workplace. Note: All nominees will be invited to attend, and a photo waiver will be included.
